People, who knows how to say in English "cadastro" and "recadastramento"?
I've found "form" meaning "cadastro, formulário" but I'm not sure because it seems that a "form" is a document that you have to fill it out with the requested information. I'm looking for a word that means a kind of document where all the information about a person is already written there.
How can I say when someone has to "confirm" some personal data in order to keep a benefit, a pension, for example? Here in Brazil, we have to do it (recadastramento) to avoid deceptions.

1)Registration=Cadastro
Ex:Do you have your registration number?(Você tem seu número de cadastro?)

2)Registration renewal=Recadastramento
