I don't know whether faz muito tempo fits into either of these contexts, but here are two examples that occur and which I hope contribute positively to Donay Mendonça's excellent answser.

CONTEXT 1:
You are late arriving to the movie theater and the film is already playing when you walk in. You ask your friend who arrived on time and who you are meeting at the movie theater,"How long has the movie been playing?" (= when did the movie performance this evening start). Or you could ask, "Has the movie been playing long?" "When did the movie start? Have I missed much?"

CONTEXT 2:
You are looking online or in the newspaper for a movie to watch. You find a title which sounds interesting. You ask your friend, "How long has it been playing?" (=when was it released, when did they start showing it in theaters).
