Ele está doido, doidinho da silva = He’s raving mad, completely crazy.

Embora fora do contexto, mas apenas para registro:

Se fosse: Ele está morto, mortinho da silva. Seria: - He’s dead, dead as a doornail.

Conservando o “da silva”.
