Cabeçalho de uma carta formal em inglês

Hi!

I'm applying for an university abroad and I need to write a presentation letter. I've finished the letter already, but I don't know for sure how to begin it, I mean, where do I write my address? On the right or left top of the page?

Is there something else i need to write in the top of the letter (page) besides my address? should I write the name of the university before the first phrase " Dear..."? And when I don't know the name of the person who will read it I just start with "Dear," ? And my name goes just on the signature, right? I don't have to write it with my address, have I?
