It took a while for me to guess what it was meant, really. If I guessed right it could be something like:

The patient is (clinically) eligible to switch from IV (or other routes) to oral meds/oral intake
The patient is now eligible to switch from IV to take medication orally.

By orally, means by mouth. So, it can be swalled, chewed or placed under the tongue to dissolve. Depending on the case.
By IV, I mean intravenous medication (intra+venous = into the vein).
