Actually there are different ways in which you can express both "ser" and "estar" in English in that context. That's what the verb "look" is for in this case, it plays the role of what "estar" is for us Portuguese speakers when you're referring to something or someone being in a particular state of physical appearance in a specific moment in time.

She's looking really skinny (ela esta multi magra).

Also the use of "ing" is important to make it clear that you mean the person you're referring to looks skinny while not having been that skinny in the past, so it's about a physical transformation they have gone through. "She looks very skinny" can give off the same idea as "She is very skinny".

"She is twig-thin". But this could sound impolite you better say "she is very skinny".

In English, in such a sentence there's no distinction between "ser" and "estar" so what is said in context will give a clue of which you mean. To solve this dilemma you could say "she seems very skinny" (these days, etc).
"It seems like this girl is not eating, she is getting very skinny by the day, and her arms and legs are looking quite bony." etc.
So, it goes with context, or some further elaborating (explaining why do you think so).
