Oi, pessoal! Alguem, por favor, poderia me ajudar a expressar esta ideia em ingles? Significa quando alguma coisa/situacao se torna emergencial ou extremamente necessaria. Thanks!

I said I would do it when hell froze over, and I hear it's getting very cold in hell. (I said I would never do something, but it appears I will have to do it.)

Acho que nao deixei claro o que a frase significa, entao os exemplos nao se aplicam (pra falar verdade nao entendi a do Marcio :? )
Muitas pessoas usam a esta expressao "quando a agua bate na bunda" para expressar que tal coisa soh eh feita qdo nao se tem mais jeito, jah se tornou emergencial.
Por exemplo:

Ela soh vai ao medico quando a agua bate na bunda.
Ele comecou a estudar porque a agua estava batendo na bunda.

Sometimes there are equivalents for slang, sometimes there are not.

"She only goes to the doctor when the going gets tough" - would that sound strange?

Yes, Flavia, it would sound strange.

When the going gets tough = When life is harsh, when you are facing lots of non-specific difficulties, when you are out of work and in poor health, when you can't find a job and you owe money, etc. It is all very non-specific. Ah.... I just thought of something: "When life gives you lemons...." By "lemons" we mean bad luck, bad health, broken romances, a death in the family, the loss of employment, etc. Very non-specific. It could refer to anything.

The tough get going - Strong people react and work hard, achieving new goals.

Para mim até a frase em português soa estranha, nunca ouvi alguém usar essa frase, apenas em um ditado que prefiro não comentar aqui :lol:. Talvez seja local.

Acho que poderia traduzir dessa maneira, mas acho que os nativos não devem usá-la, apesar de entender o que você quer dizer com a frase.
You either sink or swim.
Eu acho que a expressão mais aproximada seria "when push comes to shove".
