Acho que nao deixei claro o que a frase significa, entao os exemplos nao se aplicam (pra falar verdade nao entendi a do Marcio )
Muitas pessoas usam a esta expressao "quando a agua bate na bunda" para expressar que tal coisa soh eh feita qdo nao se tem mais jeito, jah se tornou emergencial.
Por exemplo:
Ela soh vai ao medico quando a agua bate na bunda.
Ele comecou a estudar porque a agua estava batendo na bunda.

When the going gets tough = When life is harsh, when you are facing lots of non-specific difficulties, when you are out of work and in poor health, when you can't find a job and you owe money, etc. It is all very non-specific. Ah.... I just thought of something: "When life gives you lemons...." By "lemons" we mean bad luck, bad health, broken romances, a death in the family, the loss of employment, etc. Very non-specific. It could refer to anything.

Para mim até a frase em português soa estranha, nunca ouvi alguém usar essa frase, apenas em um ditado que prefiro não comentar aqui . Talvez seja local.
