Depends on the text/exam you are sitting, or even if it´s a job interview etc.
The test may come in several formats, in pairs, a group interview (sometimes some sort of brainstorming about some matter or other), and the list could go on and on.

My experience on this one is a bit limited, I used to take an English conversation course long ago, there where a speaking test from time to time, it was taken in pairs, and each pair would choose a topic to talk about.
We would pick some sort of ballot (from a box) and open to see what it was about, with luck we wouldn't be the first...so we would have some ten minutes or so to practice.
In such events one has to try to act confident (talking in a matter of factly way, so speaking slow (but not exaggerating in the slowness) helps a lot.
As the course was a conversational one, we had a fair share of it in our classes, so in a way we were ready.

Caso vc esteja em um curso de inglês a prova será em torna da matéria abordada.
Exemplo, vc estudou o "simple past", provavelmente o professor fará perguntas que precisam ser respondidas utilizando o "simple past".
Uma entrevista de emprego será diferente, perguntarão sobre sua experiência profissional, anseios futuros, seus pontos fortes e fracos, como uma entrevista em português, no entanto, em Inglês.
