Se tivesse a opção "about", poderia usar?
Existe caso específico para usar somente "of" ou somente "about", em colocações com "to be sure"?

Muito boa pergunta!

1)Quando temos "certeza" do que sabemos,do que falamos, é possível o uso tanto de "of" quanto de "about".
Ex:I´m sure about/of what he did.(Eu tenho certeza do que ele fez.)>>Eu sei,é verdade,está correto.

2)Nos casos em que a "certeza" é de que algo vai acontecer,existir,é aconselhável e muito mais comum o uso de "of".
Ex:One thing you can be sure of,they´ll never quit.(De uma coisa você pode ter certeza,eles nunca vão disistir.)

Resumo:
Certeza do que fala,sabe,conhece,vê:Use tanto "of" quanto "about".
