São phrasal verbs, quando uma palavra preposição/advérbio modifica o verbo, nos casos abaixo fall não foi modificado drasticamente em nenhuma das três:
fall down
Just emphasizing the verb = cair no chão, desabar;
Mas, pode ter outra conotação "be shown to be not true or not good enough
And that's where the theory falls down."
fall off
To decrease in quantity or quality = cair no sentido de decrescer, diminuir;
Attendance at my lectures has fallen off considerably.
opposite rise
fall out
1 to become loose and drop = se soltar e cair pela gravidade;
His hair is falling out.
My tooth fell out.
2 if soldiers fall out, they leave their lines and move away = descer de posição o sair das linhas de frente numa batalha
3 fall out of bed = cair da cama
He was so drunk and fell out of his bed.

Vale a pena checar outras possibilidades, há inúmeras.

Addendum:
The differentiation between "fall off" and "fall out of" is much less arbitrary when we describe the physical act of falling. In this sense, it can be said that:
People or things fall out of places which they are in. E.g.:
Last night I fell out of bed (because we lie in bed).
The construction worker fell out of the elevator (enclosed space).
People or things fall off surfaces which they are on. E.g.:
Be careful not to fall off (the roof).
The vase fell off the shelf.

If you slip or trip, you might fall over:
I slipped on the ice and fell over
I was running and I tripped and fell over.
You fall down the stairs:
I slipped and fell down the stairs and broke my leg.
You fall off something you are standing on or riding:
she fell off a motorbike and was lucky to survive
