Meaning 'spirit/ghost' (aparição/espiríto/assombração) those words are interchangeable.
"Ghost" have other other (and more) meanings, including metaphorical ones - as in "ghost write", ghost - andar como um gato/furtivamente/como um fantasma (sem fazer som, aparecendo do nada na sua frente), a phantom of the past (um fantasma do passado - que volta para lhe assombrar/perseguir, um segredo ou algo ruim que não se quer lembrar.), etc.

Among ghost/wraith/phantom:
Ghost is the most common for general use.
Wraith is much less commonly used in AmE, though it can mean largely the same things as ghost, according to the dictionary. I literally only know this word through Starcraft and Lord of the Rings.
Phantom, when referring to a ghost, has a more sinister connotation to it, like a ghost that is specifically haunting or attacking or stalking someone/something/someplace.
They all have figurative or side-senses aside from the literal apparition sense, which are all slightly different, and which you can read about in a dictionary.
