There is a big difference between the words house and home. A house is a structure or the physical place where you live in. A home is more of abstract or sentimental but can also be a physical place. Think of it this way. You use house to describe the actual place but you use home when you describe the place where a family or a person lives.

Explicando um pouco...

Quando falamos no lugar onde nós vivemos, dizemos que é nosso lar, independente se é casa ou apartamento, portanto usamos "home" quando falamos de nós mesmos ("my home", "I'm going home", "this is my home", etc.)

Quando falamos onde o ouvinte ou uma terceira pessoa vive, falamos sobre a casa ou apartamento desta pessoa, não falamos em lar, portanto se usa "your house" ou "your flat", etc.
Uma exceção a isto é quando perguntamos onde alguém mora, aí podemos usar "Where's your home?", especialmente se não soubermos se é casa ou apartamento.

Porém, ao generalizarmos (como em ditados populares, por exemplo), dependendo do contexto também se usa "home", como, por exemplo, no ditado "Home is where you can hang your hat" ("(Um) Lar é onde você pode pendurar seu chapeu", em tradução literal)
