But I can help clear up a few things:

To out someone = explanar alguém, divulgando um fato desconhecido/escondido da vida dele.

I feel like I need even more context for this, I'm not sure why he would be hiding the fact that he was "out of country" in a medical context.

When "why don't you" is at the end of the phrase like that, it is a sarcastic admonition, meaning that he didn't want the person to "out him as out of country".

Without much context it´s a challenge...!

But okay, my first educated guess would be something like: "Sim, do médico e do país, e por que você não (tem cicatrizes? - talvez haja um certo sarcasmo envolvido - sem cicatriz, sem ter servido ao país?).

Out of - also means "motivaved by" (see WordReference) that was my reasoning here. So, "out of me" (motivadas por eu ser médico) as (assim como) motivadas pelo país [pelo servir ao país]

Another possibility is the patient having a scar, then the irony or sarcasm would be on the part of him (the patient). Then the medic retort back with:
"sim, é minha (foi eu que provoquei/deixei acontecer, etc - por causa do meu atendimento, do meu serviço.)

Then: e do país (o país também tem culpa por não oferecer o material necessário para um atendimento melhor, que não deixe cicatrizes).

Finally, the "why don´t you" (no segundo contexto poderia ser que ele pensou num impropério mas deixou à meio caminho "por que você não vai...").

Could be the patient talking about the scar (that of the patient), perhaps a small one, hence the (good) surprise.
Could be:
"sim, você tem essa cicatriz graças a mim (à cirurgia que fiz) tanto quanto graças ao seu país Aprecio (e aceito) o elogio, mas tudo que fiz foi pegar uma serra cirúrgica. Tudo bem que eu savei minha vida (também) pois senão meu status/meu prestígio/reputação como médico iria lá embaixo."
