If this is from a TV show or podcast or something, could you tell us which one and which episode?
But I can help clear up a few things:
To out someone = explanar alguém, divulgando um fato desconhecido/escondido da vida dele.
I feel like I need even more context for this, I'm not sure why he would be hiding the fact that he was "out of country" in a medical context.
When "why don't you" is at the end of the phrase like that, it is a sarcastic admonition, meaning that he didn't want the person to "out him as out of country".

COMO COMBINAR PALAVRAS EM INGLÊS
Without much context it´s a challenge...!
But okay, my first educated guess would be something like: "Sim, do médico e do país, e por que você não (tem cicatrizes?
Out of - also means "motivaved by" (see WordReference) that was my reasoning here. So, "out of me" (motivadas por eu ser médico) as (assim como) motivadas pelo país [pelo servir ao país]
Another possibility is the patient having a scar, then the irony or sarcasm would be on the part of him (the patient). Then the medic retort back with:
As I said, without context...!

Ok, aqui está a frase completa:
"Yeah, just out me as out of country, why don't you. I appreciate the compliment, but all I did was catch a wild buzz saw. Sure, I saved MY life but my status as medic went down."

Could be the patient talking about the scar (that of the patient), perhaps a small one, hence the (good) surprise.
Could be:

Thank you Bryan for you invaluable insight on this one, we all appreciate. English is indeed a vast realm, as any language is.
