Do you have one?
"Do" aqui serve para fazer perguntas (usa-se com verbos que não sejam "to be".
You do have one?
Você realmente tem um?
You do have one, don´t you?

Using DO in the beggining of the sentence it would be just a question - without emphasis. This emphasis to express doubt, incredulity, surprise, etc.
