I see that in all of your 12 postings you used Portuguese, so that could suggest that is a habit that gets in the way of building up vocabulary.
Maybe the issue is that you took too seriously the "note" part of 'notebook', only learning by rote (being able to repeat rather than understanding it).
Just taking notes, sort of, is an issue - because we don´t grasp the nuances of corporal language, of the description of places, words, and expressions that convey stronger agreement or disagreement, etc.
When I mention words of agreement/disagreement, I also mean the discussion, the interaction with others and show your ideas, point of views and share insights (not only about English the study subject, but how to fix a coffee, how to give directions to a stranger that wants to get to the bank, etc).
Doing that, one thing leads to another, your brain begins to make associations, establish contrasts and similarities between words and you enrich your vocabulary repertoire with new expressions.
By doing the watching/listening you can tell one accent from another, you can tell colloquial and informal language from formal. Your "ear for English" develops and even your pronounce gets better.
By reading a mix of texts belonging to a variety of genres and interpreting them would enable you to be more skilled in English (I mean to express yourself). And there are sites with exercises of interpretation of texts (and books too).
If you like music. You can make use of it to have fun and learn some different pronounce of a word (heart - for instance - there are at least two ways they pronounce the heart part).
The vocabulary notebook? Don´t throw it away, with time you will need it less, or those words will become so commonplace that you will give way to new ones, along with new expressions.
Finally, I am not saying that you can´t use Portuguese, what I mean is, you can put more English in your life and in studies. It pays off.
I didn´t cover everything, others might come up with other perspectives on learning and share their experience, let´s wait them comment on.

Keep up the good work and good luck with your English studies. Count on us.

One other thing is the paralysis analysis.

The “best” language learners are actually the worst ones
They need to know as many words as possible before they dare try to use them with a native. What if the native casually mentions his pet budgerigar and you haven't learned that word yet? Then everyone will point and laugh at you!

And they need to have grammar so tight that even the most pedantic university professor wouldn't be able to critique them.

As you see, my English is less than perfect, sometimes my writing is shoddy and knowledge of grammar far from perfect. My vocabulary repertoire a bit limited (however much I study and try to build it up).
It's what I have, if I make some mistakes so what? I get corrected and move on, at least I have learned one more thing.
We have to embrace our mistakes and weakness, not that we will make a point of being wrong all the time!
When we can't do the best we do our best, is that simple!
There are learners that think they will learn by osmosis, will they? No. One can't just transfer knowledge as if there was a Bluetooth link, from one brain to the other. They didn't found this breakthrough yet.
Okay, I admit sometimes I get a bit unsure about some word, then I look it up elsewhere. When in doubt, I change the wording altogether. And the show goes on. :-)

Some time ago I used to study in the following way, an allocated time (minutes) to listening, then grammar (with exercises), then reading (a novel for instance - or reading some newspaper online, etc), reading and interpretation (now using a textbook with answer key - but no cheating). Sometimes I would have a day for texts/exams, etc.
One can also divide those activities through days, Mondays - Grammar, and having fun (reading, then music, etc). Tuesdays - Tests/exams, and so on.
Each student has one method that fits his learning, this one was just for illustration's sake.
Thanks a lot man! You really helped me out here.
Yeah, this is a serious doubt that I have. If there's such a thing like this "osmosis learning process" which I've heard a lot around there.
The teachers on youtube that says that you just have to "live in English" are too numerous to list. But then, it's never been clear how should I do that. They say things like "Go on and listen to musics, watch movies and tv-shows" and so on. But there's a thought that always comes to my mind, like "Ok, but how do I organize that?
I really can learn English without taking notes?"

You advices kind of answered this questions for me. Thank you very much.

Glad that it was of help.
One more thing: we first learn how to make sentences/to express ourselves in English. Then we learn how to express ourselves in more ways than one, that´s one sign that we are improving a bit.
That is not "the one" sign, though. Just one of them. Another sign would be when we make a mistake, we be able to correct ourselves.
Why don't you try to take it up a notch by using Anki? I've been using Anki for almost 3 years and it has improved my study performance a lot. I'll describe roughly how it works for me:

I watch a TV show, a youtube video or listen to a podcast for about 1 hour a day. Every single word that I either don't know the meaning or is not part of my vocabulary I'll put down on a notepad. It can be five, twenty, thirty words in a day, doesn't matter. Later, I will look up the meaning of this word, find at least three sentences where it appears and add to Anki.

It's a cycle. First thing in the morning, I will review all my cards, and then it repeats over and over. Anki does the job of showing me only the cards that I need to see on that specific day, in some kind of memorization cycle. That's the best way I know to retain words.

I also put aside some minutes each day to read an article or anything that tickles my fancy. For example, today I read an article with the title "7 Reasons to Dream Big".
I put the sentences on the Google Translator and try as hard as I can to make out the very details of each word. As long as I don't feel that I'm pronouncing like a native, I won't stop.

Honestly, I don't know if my method is appropriate for anyone, but it works for me. My writing and communication skills are not great and you're likely to find errors in this text. I don't practice these skills as much as I'd like. I use the free version of Grammarly to correct my writings. For example, Grammarly usually tells me that the length of my sentences are above average.

Anyway, I think that there's no endpoint when it comes to learning, even languages.

Thanks for sharing your experience and for your suggestion as well.

In fact, I just found minor things to get corrected. Like where it reads "doesn´t matter" should read it doesn't matter. Some punctuation could be redone, but nothing that would be overly dramatic, so it´s okay as it is. One can understand your idea.
Overall, it´s pretty good. See? You were sweating over the small thing!
And yes, languages learning is a continuous process, even our language is tricky and puzzling sometimes. Perhaps this is the beauty of them, ha ha ha! Just kidding...
