Supondo que eu queira perguntar a duas atrizes de teatro sobre o por que as duas nunca atuaram juntas. Detalhe: uma está aposentada e não poderá mais atuar.
Uma possível tradução minha - e quero no pretérito perfeito - é a seguinte:
Why did you and she have never been acting together?
Porém, não seria 'she has never been'? O 'did' altera o 'have' para 'tinham'? Ou seria 'had'? Haveria outras formas de dizer?

Why did you never act together?.... Ou
Why didn´t you ever act together?
* POR QUE VOCÊS NUNCA ATUARAM JUNTAS?
Dá uma ligada no YOU (vocês = plural), se complicar, manda um BOTH, TWO ou GUYS após o YOU que tudo fica numa nice!

You can use Simple past, the way Redseahorse answered.
However, if you use Perfect tense, never use did + Perfect Tense
Why haven't you been acting together? (until now - esta é a idéia do perfect aqui).
Why haven't you acted together? (the same play "peça" or film)
Why have you never acted together? (também é opção)
Perceba que posso reduzir you and she para you = vocês
