It depends, I´ll give some examples to illustrate the points (indeed "sempre" can be taken literally or just a way of saying.
To an adolescent you could say "Are you like that all the time? seems like you are angry and hate everything and everyone..."
(just kidding around, one knows that this happens because teens want to be accepted and find their way, their space, we are the ones that don´t find time to listen them, sometimes.)
What´s the matter with you? Why are you so grumpy/grouchy all the time?
Why are you irritable/angry/bad-tempered/with a bad temper all the time?
Why you get angry a lot for no reason?

But you may want to be polite, or you ''feel" that the person is experiencing some touchy feelings, some rough patch in life, etc.
You might say something like:
Is it my imagination, or is something going on here? want to talk?
Is there something that makes you umcomfortable with?
...Since sometimes if you say someone is angry, it will irritate him/her even more!
One way to irritate someone is asking if he/she is mad/angry at you? etc.
