In medicine, a disposition plan outlines what happens to a patient after their initial treatment or assessment.

In law - the outline of how the Will is going to be distributed on death.
In Gardening could the be where the sprinklers, fors example, would be positioned. Having to do with layout.
In medicine - the next step(s), something like "diretrizes/instruções após, ao ter ou para alta hospitalar".
Não exatamente alta médica. Alta hospitalar é ao ser liberado de um hospital, e médica é quando se está curado ou não precisa "follow-ups", etc. It would be "(esquema ou) procedimento(s de alta hospitalar" or "procedimentos após a alta hospitalar", etc.
In technology/computer - could be "plano de descarte", of equipment parts, etc.
Granted, in "medicine" here its more of a description than a technical term. I admit my knowledge is a bit limited on this field.
