Eu vi uma foto desta tal pessoa no facebook e eu gostaria de perguntar se é ela.
Posso perguntar: It's you? / "Is that you?" (esse eu já ouvi personagens em séries perguntarem)

"Is it you?" ou de maneira mais informal "It's you?"

Yes, in a generical way. In a way it means that you are making "an educated guess", you expects that it will be some. You might be "guessing" by the voice on the other end of the phone, or you are waiting for someone and he/she knocks on the door (or it´s highly problable that he/she has just come.)
Is that you? would be a little more formal, I think.
