Both are common and formal.

I recommend against the use of the other terms you found.
to make a date = very informal, can be confused with a "programa romántico"
We appoint people, not dates and meetings.
to fix up = can be confused with a "programa romántico"

Arrange

• Matthew arrived at 2 o'clock as arranged. [Matthew chegou às 2 como foi agendado/combinado.]

"I'd like to make an appointment with Doctor X, please".

"I made a dentist appointment for Friday. Hope that's okay with you".

You could also set up or arrange an appointment.

Ah! Seria todos os termos dos posts anteriores, mas você pode usar do Native Speaker Thomas - veja:

1) Let's set up an appointment for one hour the next time/meeting.
[No caso para uma consulta médica.]

2) Yesterday how the class was scheduled for two hours, let's arrange this class (or new class) at 1:00 pm for one hour today.

Yes, Mellissa sentence #1 (number 1) is fine to me.

Whereas with #2 I would redo it a bit:
Since yesterday´s class was scheduled to begin at 2 p.m. (others would prefer 2 P.M. Here) I would like to schedule for the next lesson/class to start at 1 p.m. (today)[1].

I could suggest "today´s class" instead of "next class" but I think if you do that you have a tight schedule... And the other party (a tutor/teacher) maybe will have a hard time to adjust his/hers to it.

And, of course, there are other ways to express that.
