A note has been left to him.

3. She is telling a fairy tale to the children.( active voice)
A fairy tale are being told to the children.
The children are being told a fairy tale.
4. Somebody sent chocolates to Susan.( active voice)
Chocolates were sent to Susan.
Susan was sent the chocolates
