Sometimes it goes with the context, a spare could mean something kept to be available in an emergency "a spare tyre/a spare room", etc. So, it would be defined by the purpose here.
Extra - in some cases means "additional" (in the sense you can optimize or even decrease/get rid of something as a trade-off ) as in "if you put add weight in your car it will consume extra fuel."
I mean, aditional fuel, not 'spare' fuel, but if you unless you keep a Jerry can with fuel for an emergency, it would be a spare fuel container. Somehow "spare" would mean "redundance" in a system.
Again, if you read an ad "the lunch doesn't come with coffee and dessert. You may order for extra charge." That would mean that coffee "isn't included", so "extra" would mean additional/supplementary/added charge.
And, as RedSeaHorse has pointed out, more often than not they are interchangeable.
