Usamos as expressões geralmente pra discordar de alguém, geralmente um absurdo, algo sem noção:

What are you smoking?

Exemplos:
What are you smoking? You have obviously never lived in a true third-world country if you believe that statement.
She´s the prettiest girl in the school? What are you smoking?

What the hell are you talking about, are you on drugs??

Do que é que cê tá falando? Tá doidão?/Tá chapado?/Andou fumando umas??
