"Cocoa" is a drink that is primarily chocolate. It can be mixed with cold milk or with hot water/milk.
"Chocolate" can be a drink, but it is often a flavor, a candy, and ingredient for a cake, etc.

If you asked "Do you want some cocoa", I would understand that you are offering me a drink, and I would ask if it was going to be hot or cold.

If you asked "Do you want some chocolate", I would understand that are are offering me some chocolate candy.

Both mean... or both of them mean...

Subject-verb agreement -- Since "both" refers to more than one item, then it will take a plural verb.
