It´s worth noticing (to people at large, the ones that don´t watch medical/hospital shows) that "a condition" can be translated into "um problema (de saúde)/um problema médico/uma condição médica/uma condição clínica (ultimamente alguns teem traduzido assim)".
A more technical approach would be "uma patologia (médica)".
