Lots of meanings.

Someone could be a fixer and work out things for you.
Someone could interced for you.
Someone could go to someone else and ask something for, or try to persuade him to do something to you.

And other ways, certainly more members of the EE will chime in and offer their comments.

Mediate would be used more to refer to "mediação", which is a word that suits more to cases when the two parties are litigating, fighting or there´s some kind of misunderstanding between.
In that case, it´s (more or less) supposed that the two (or tree are equal in power, rank etc.), to cite an example from religion, a believer would seek intermediation by Jesus, a saint, an angel, etc to intermediate between God and men, in this allusion one understands that God is higher in rank and power, etc.
Mediate would be, for instance, two nations at war need a truce or some peace talks, they may seek a legal board (made up of jurists, statemen, etc of several countries), so one is chosen to mediate the talks (between the two countries). It seems like it is a bit different of "mediação" in Portuguese, that sometimes can have both meanings.
