When I really want something, I'll go through hell and back (to get it) if that's what it takes.

When I really want something, I'll stop at nothing to get it.

Depending on the context, we have the possbile anwers:

(1) When I want something badly enough, I'll go to Hell if I have to.
When I am badly in need of something, I'll go to Hell if I have to
>> Quando (eu) estou desperado(-a) por algo, vou ao Inferno se preciso for.
>> Quando (eu) preciso muito de algo, vou até o Inferno se preciso for.
>> Quando (eu) quero muito alguma coisa, vou ao Inferno se preciso for.

Note: We capitalize the words Heaven and Hell, when they are used as proper nouns (i.e. As names of specific places).
