"Lean" is mostly used when something is resting against something, supporting something, normally on an angle.
"Lean the ladder against the wall."

It can also be used to describe something that is not straight but on an angle.

"Tilt" is used to show that something has been moved or is going to be moved to a sloping position or to another angle.
